Admission Procedures

Requirements for New Students A.Y. 2026-2027

PRE-KINDER & KINDERGARTEN

• Accomplished Application Form
• Two (2) recent identical 1x1 ID pictures
• PSA Birth Certificate (original and photocopy)
• Baptismal Certificate (original and photocopy)
• PRE-KINDER applicants must be 4 years old by December 31, 2026
• KINDER applicants must be 5 years old by December 31, 2026

GRADE SCHOOL

For Grades 1-6:
• Accomplished Application Form
• Two (2) recent identical 1x1 ID pictures
• PSA Birth Certificate (original and photocopy)
• Baptismal Certificate (original and photocopy)
For Grade 1:
• Applicants must be 6 years old by December 31, 2026
• Certificate of Kinder Completion
For Grades 2 to 6:
• Original and certified true copy of Report Card (SF-9/F138) with a satisfactory general average and conduct grade
• Two (2) Recommendation Forms to be filled out by any two of the applicants' present or previous advisers, subject teachers, principals, or guidance counselors/associates

JUNIOR HIGH SCHOOL

For Grades 7-10:
• Accomplished Application Form
• Two (2) recent identical 1x1 ID pictures
• PSA Birth Certificate (original and photocopy)
• Baptismal Certificate (original and photocopy)
• Original and certified true copy of Report Card (SF-9/F138) with a satisfactory general average and conduct grade
• Two (2) Recommendation Forms to be filled out by any two of the applicants' present or previous advisers, subject teachers, principals, or guidance counselors/associates
For Grades 8-10 Grantees Only:
• System generated Educational Service Contracting (ESC) Program Transfer Out Certificate

SENIOR HIGH SCHOOL

Open for Grade 11 Only:
• Accomplished Application Form
• Two (2) recent identical 1x1 ID pictures
• PSA Birth Certificate (original and photocopy)
• Baptismal Certificate (original and photocopy)
• Original and certified true copy of Report Card (SF-9/F138) with a satisfactory general average and conduct grade
• Two (2) Recommendation Forms to be filled out by any two of the applicants' present or previous advisers, subject teachers, principals, or guidance counselors/associates

Application Procedures

• STEP 1

  Submit complete requirements together with the accomplished application form for evaluation to the REGISTRAR'S Office



• STEP 2

  Secure the Application Data Entry Number to be issued by the REGISTRAR'S Office after the evaluation of requirements.



• STEP 3

  Pay the admission processing fee of P500.00 to the CASHIER



• STEP 4

  Present payment receipt to the GUIDANCE Office for the schedule of the intake interview with the Guidance Counselor/Associate and the Assistant Principal for Academic Affairs (Preschool to 10 & Senior High School)

Grade School (Gr. 1 - 5)	Every Wednesday and Thursday

Senior HS (Gr. 11)	Every Friday

- Intake interviews shall be conducted on the same day by the Guidance Associate/Counselor and the Assistant Principal for Academic Affairs.
