ADMISSION REQUIREMENTS
FOR NEW STUDENTS A.Y. 2025-2026

1. PRE- KINDERGARTEN
   1. Applicants must be 4 years old by September 30, 2025
   2. PSA/NSO Birth Certificate (original and photocopy)
   3. Baptismal Certificate (original and photocopy)
   4. Two (2) recent identical ID pictures (1 x 1)
   5. Accomplished Application Form (download from the school website)
2. KINDERGARTEN
   1. Applicants must be 5 years old by September 30, 2025
   2. PSA/NSO Birth Certificate (original and photocopy)
   3. Baptismal Certificate (original and photocopy)
   4. Two (2) recent identical ID pictures (1 x 1)
   5. Accomplished Application Form (download from the school website)
3. GRADE SCHOOL & JUNIOR HIGH SCHOOL
   1. Grade 1 applicants must be 6 years old by September 30, 2025
   2. Original or certified true copy of Report Card (SF-9/Report Card) with Learner’s Reference Number (LRN)
   3. A General Average of at least 83 %
      • For early applicants: submit at least the first two quarters or the first trimestral grading period of the current school year
      • For end-of-school-year applicants: submit the final grading period of the current school year
   4. A Conduct Average of at least 85 % or B
   5. PSA /NSO Birth Certificate (original and photocopy)
   6. Baptismal Certificate (original and photocopy)
   7. Two (2) Recommendations from the Principal or Class Adviser and Guidance Counselor/Associates of the previous school
   8. Two (2) recent identical ID pictures (1 x 1)
   9. Educational Service Contracting (ESC) system generated certificate for Grade 8 to 11 applicants
   10. Accomplished Application Form
4. SENIOR HIGH SCHOOL (Grade 11 only)
   1. Accomplished Application Form
   2. PSA /NSO Birth Certificate (original and photocopy)
   3. Baptismal Certificate (original and photocopy)
   4. Two (2) Recommendations from the Principal or Class Adviser and Guidance Counselor/Associates of the previous school (use LSQC Form)
   5. Two (2) recent identical ID pictures (1 x 1)
   6. Educational Service Contracting (ESC) system generated certificate for Grade 8 to 11 applicants
   7. Original or Certified True Copy of the latest Grade 10 Report Card (SF-9) with:
      • - a general average of at least 83% in at least the first two quarters or first term grading period of the current school year
      • - A conduct average of at least 85% or B

APPLICATION PROCEDURE

• Step 1:
  Submit complete requirements together with the accomplished application form for evaluation to the REGISTRAR’S OFFICE
• Step 2:
  Secure the Application Data Entry Number to be issued by the Registrar’s Office after the evaluation of requirements.
• Step 3:
  Pay the admission processing fee of P500.00 to the CASHIER
• Step 4:
  Present payment receipt to the GUIDANCE OFFICE for the schedule of the intake interview with the Guidance Counselor/Associate and the Assistant Principal for Academic Affairs (Preschool to 10 & Senior High School)

SCHEDULE OF INTAKE INTERVIEW

- Intake interviews shall be conducted on the same day by the Guidance Associate/Counselor and the Assistant Principal for Academic Affairs.
